In order for us to provide and expedite the best possible care for you, it is very important that you bring the following items to your scheduled visit:

New Patients:

• Driver’s License or Photo ID
• Insurance card
• Patient Forms
• Medical Records pertinent to the topic of concern

Returning Patients:

• Driver’s License or Photo ID
• Insurance card

 

All office visits are payable at the time of service. We accept cash, checks, and most major credit cards and debit cards. Where we participate with most insurances, we will bill your third party carrier for services rendered.  Once the insurance carrier determines a patient’s cost-sharing responsibility, we will issue a statement to your billing address. Please note that if your insurance does not cover services provided, you are responsible for this payment. It is the responsibility of the patient to keep Franklin Medical Center up to date with all insurance and billing information. Please contact our office at (732) 851-7007 should you have questions or concerns.
